Please use the form below to submit your request for access to the "LabMaterials" and/or "LabSubmissions" systems.
You can list multiple unit codes in the "Your unit code" field. If you have more than one staff member wanting access, they'll need to complete this request form as well. Please ensure you read the "How To Guide", and maintain backups of the materials/LabSubmissions.
